The aim of PEP
Is to improve your health and wellbeing by providing a route to organisations, activities and services.
PEP link workers will help you to:-
- Decide the things you want to change
- Create a plan to make those changes
- Give suggestions and information about services who can help
- Make referrals to the services who can help
PEP is a free service for anyone who is over 18 and registered with a GP Practice in West Leeds. PEP is short for Patient Empowerment Project, which means that you make the decisions and we help you find a way to get there. Often when you are unwell you will need to go to your Doctor for a prescription but sometimes there are other things you can do to help yourself feel better, such as making changes to your lifestyle or dealing with the things that cause you stress.
Sometimes it helps to be with other people who have similar experiences to you. The PEP team are employed by 3 different organisations; Barca-Leeds, Better Leeds Communities and Leeds MIND. They come from a range of backgrounds and have a great deal of knowledge of services that are available so can make finding the right thing for you much easier. Someone from the team can visit you at home or at the surgery to listen to your concerns and explore different options to help you manage your own health or do the things you would like to do but don’t know where to start.
We are currently holding a fortnightly referral clinic based on alternate weeks at Glenlea Surgery. This will give you an opportunity to meet with a member of the PEP team face to face to complete an initial referral to the service. If you would like any further information about this or would like to book in an appointment to make this referral, then please speak to your GP or a member of staff at the surgery who can guide you further with this process.
